Remote Patient Care Pharmacist – Virtual Counseling & Adherence SupportProvide direct medication counseling, adherence support, and clinical monitoring through telephonic or video visits — all from home. Key ResponsibilitiesDeliver telephonic or video-based medication counseling and education to patients and caregivers.Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.Conduct medication reconciliations and reviews, especially for high-risk or complex patients.Collaborate with physicians, nurses, and case managers to coordinate treatment plans.Document all interventions and communications according to organizational standards.Support population-health and disease-management programs.What You'll BringEducation: PharmD or Bachelor of Pharmacy.Licensure: Active U.S. pharmacist license (multiple state licenses preferred but not required).Experience: Ambulatory care, MTM, or clinical preferred.
